Case Manager Facilitator (MH Counselor II) Wraparound Bilingual Spanish

Facilitates/manages Child & Family Teams and facilitates teaming with agency and community partners
Facilitates the Child and Family Team and assures the ongoing development of individualized service CFT, safety, and transition plans for assigned cases.
Develops Wraparound Plans of Care and Safety Plans, and all required paperwork.
Leads the strengths-based, family-centered, assessments of the child and family team and assures that all involved with the case contribute to the assessment.
Coordinates, schedules, and facilitates the child and family team meetings.
Assures that planned services are provided.
Facilitates the self-assessment of the team.
Participates in the Quality Assurance Program.
Develops and provides specific services identified in the individualized service plans.
Provides crisis intervention as needed.
Collaborates with parent advocates, psychiatrists, and network providers.
Assists in the preparation of quarterly and annual reports.
Prepares state and county wraparound paperwork.
Performs other duties as assigned.
  • BA in psychology, or related field

  • Two years experience working with children and their families.

  • Cleared by Department of Justice (Child Abuse Index).

  • Possess valid class C California Driver’s License and driving record meets SFVCMHC, Inc.’s insurance requirements.

  • Culturally sensitive and competent.

  • Bilingual Spanish/English preferred.
