Mental Health Counselor I - Adult Full Service Partnership

Provide individual crisis intervention and supportive counseling with intensive case management. Provide evaluation; plan development; placement services; assistance in daily living; linkage and consultation; group counseling to homeless mentally ill individuals.
POSITION RESPONSIBILITIES: Provide direct and indirect client services.
Participate in assertive assessment and evaluation of seriously emotionally disturbed, mentally ill individuals. Determine individual’s strengths, needs and resources. Assess client’s level psychosocial impairment, substance abuse history, self-care potential, family and support network availability, independent living needs, financial status, educational, employment or training needs, and social/recreational needs. Develop and coordinate service plans.
Complete all documentation within mandated time frame and in compliance with county, state, and Center guidelines.
Assist clients in developing or maintaining the daily living skills needed to implement and achieve the goals of the individual service plan.
Provide crisis intervention – intervene with the client/others at the onset of a crisis to provide support and assistance in problem resolution and to coordinate or arrange for the provision of other needed services.
Transports clients to and from appropriate service as necessary.
Will work as part of a multi-disciplinary team; participate in staff and clinical meetings and other meetings as assigned.
Provide individual, group, or family intervention to clients as needed, within the scope of practice.
Will perform other duties as assigned.
Must act in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements. Must successfully complete the Center’s HIPAA training, pass the HIPAA test, and receive the Certification of Compliance.
Regular attendance is an essential function of the job.
Interacting with other employees at work is an essential function of the job.
Arriving at work on time and not leaving early is an essential function of the job.
Must have a good driving record and must be insurable by Center’s insurance company.
Bilingual, Spanish preferred.
B.A. in field of social work, counseling, psychology, or related field.
Minimum of two years experience in a behavioral health care; experience with chronic mentally ill and homeless preferred. Familiar with DSMIV preferred. Case management experience necessary.
