Program Coordinator - P.A.T.H.

Duties: 

Provide direct services to assigned client caseload. Engage assigned client cases with appropriate case management, CCCP, Client Care Coordinated linkage and referral and housing stability.

Oversee and delegate intake assessments to Street to Home Clinical Intake team and provide resources and to assertive community outreach and attend community meetings.

Conduct weekly case disposition for the team and facilitate weekly client rosters for review/monitor HMIS data entry.

Review staffing, complete clinical/non-clinical incident reports, prepare monthly Board reports for programs.

Attend all QA/Center meetings and attend all meetings as directed by Assistant Director.

Review monthly productivity.

Proficient in completing all Center and DMH/Department of Mental Health paperwork.

Submit Data tracking logs to Management team by the 1st of each month.

Monitor progress towards required housing outcomes.

Ensure appropriate enrollments, outreach and follow up.

Assist with outreach, intake and enrollment.

Other tasks as assigned by Assistant Director, Homeless Services.

Qualifications: 
EDUCATION:
Master of Social Work (MSW) from an accredited school of social work AND a current Licensed Clinical Social Worker (LCSW) registration through the California Board of Behavioral Sciences; OR
 
Masters Degree (MA/MS) in psychology or related behavioral science field from accredited university AND a current Marriage and Family Therapist (MFT) registration through the California Board of Behavioral Sciences
 
Minimum of four years’ experience in a behavioral health care; experience with homeless chronic mentally ill. Knowledge of DSMIV required. Co-occurring disorders experience preferred.
 
ADDITIONAL REQUIREMENTS:
 
Must act in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements. Must successfully complete the Center’s HIPAA training, pass the HIPAA test, and receive the Certification of Compliance.
Regular attendance is an essential function of the job.
Interacting with other employees at work is an essential function of the job.
Arriving at work on time and not leaving early is an essential function of the job.
Must have a good driving record and must be insurable by Center’s insurance company
