PROGRAM COORDINATOR - TAY FSP

Duties: 
Under the direction and supervision of the Manager of TAY FSP, FCCS and CAPPS programs, the Program Coordinator coordinates and supervises the daily functions and general operations regarding the TAY FSP and TAY FCCS Programs. Additionally, the Program Coordinator provides direct service and functions as part of a mobile case management team which conducts outreach and engagement activities, provides mental health evaluation/assessment and a wide array of mental health/rehabilitative services/case management services/crisis intervention and emergency response to all FSP enrollees and FCCS clients. The Coordinator’s role is to ensure that FSP enrollees and FCCS clients receive services in keeping with a pro-active “Assertive Community Treatment Model” (ACT). The Program Coordinator also ensures that each client has timely access to needed services.
 
RESPONSIBILITIES:
 
Conducts daily team meetings in order to review each client’s care plan and staff assignments. Assesses potential crisis needs of enrollees and develops and implements intervention plans in order to prevent, de-escalate, or minimize crises situations.
 
Provides daily consultation and support to staff in assessing adequacy and appropriateness of each client’s living or funding resources and assists in securing alternative living or funding arrangements as needed.
 
Provides programmatic supervision to Team Leaders and clinical supervision to clinicians.
 
Supervises both the assessment and evaluation of seriously mentally ill transitional age youth. Determines individual’s strengths, needs, and current resources. Assesses their level of psycho-social impairment, self-care potential, family and support network availability, independent living needs, financial status, educational, employment or training needs, and social-recreational needs.
 
Carries a small caseload.
 
Ensures that clinical staff provide comprehensive service plans which specify the treatment services, activities, and assistance needed in order to accomplish the objective (s) identified, resulting from the collaboration between enrollees and case managers.
 
Develops augmented services for families and individuals being served in the Programs.
 
Provides crisis intervention and emergency response as needed and intervenes with clients/others at the onset of a crisis in order to coordinate and arrange for provision of needed services.
 
Coordinates, supervises, provides oversight to on-call staff and ensures that services are available twenty-four (24) hours, seven (7) days per week.
 
Licensed Coordinator must become LPS designated and conduct 5150 assessments twenty-four (24) hours and seven (7) days per week as needed in both the clinic and the field on a rotation basis.
 
Under the direction of the Manager of TAY FSP/FCCS and CAPPS Services, the Program Coordinator ensures that all services are provided in keeping with the standards established by both the Center and the LACDMH.
 
Ensures that all documentation regarding clients meets Center standards. Participates in Quality Improvement and Utilization Review activities. Completes and provides statistical/reports as assigned. Attends the Center’s Quality Improvement Meetings on a bi-monthly basis.
 
May be assigned to provide coverage for Program Manager in the event of his/her absence or unavailability.
 
Attends in-service training as assigned and appropriate.

Must have a California State license in good standing or possess a current waiver or registration with the Board of Psychology or the Board of Behavioral Sciences. Waiver or registration must be in good standing.
 
Minimum of two (2) years experience in a mental health setting. Intensive case management experience with the target population is necessary.  Current NPI number.  LPS designated.
 
Must complete application for “Custody Facilities Security Clearance” and be able to obtain security clearance through the Los Angeles County Sheriff’s Department.  Obtain DOJ clearance.
