Cornerstone FCCS

The program provides field-based services which includes a comprehensive array of mental health, intensive case management, dual diagnosis services and medication support services.  The program is open seven days per week and has a milieu setting which includes laundry facilities, shower facilities, locker facilities and daily mental health groups.

Target Population: 

The Cornerstone Field Capable Clinical Services program serves adults aged 26-59 with a serious mental illness who are homeless, at risk of homelessness and are unserved, underserved, or inappropriately served. Focal populations include; 1) chronically homeless, 2) serious risk of homelessness 3) recently released from jail, or high risk of incarceration; 4) multiple psychiatric hospitalizations and recent suicide attempts, and 5) have experienced trauma or are currently in abusive relationships.

Diversion 50

(LA County 3rd District Diversion & Alternative Sentencing Pilot Program)

This program is based on a “Housing First Model” which serves chronically homeless adults with serious mental health illness who are released from jail. Multidisciplinary mental health treatment is provided to clients to help stabilize, integrate and prevent recidivism in the community.

Street to Home

Street to Home is a unique program designed to outreach the most chronically homeless individuals with tri-morbid conditions, including, severe mental illness, substance abuse and chronic medical conditions, and to get them off the street and into permanent housing within 90 days.

The target population for the project is those individuals who are chronically homeless and have tri-morbid condition which is a combination of mental illness, substance abuse and debilitating medical condition.  Those with tri-morbid conditions are at the greatest risk of mortality and therefore, are most in need of intensive, field-based services.

Milestones (AB 109)

The program utilizes recovery principles to stabilize and reintegrate post incarcerated, homeless, mentally ill adults back into mainstream society while providing comprehensive support services to increase self-sufficiency and quality of life. Macdonald Carey East Valley Mental Health Center

The Macdonald Carey East Valley Mental Health Center is an outpatient mental health rehabilitation program, which provides both a comprehensive and integrated array of Mental Health Services, Targeted Case Management Services, Medication Support Services, and Crisis Intervention Services to adults with a severe and persistent mental illness.  The program’s mission is based upon the psychosocial rehabilitation model, where principles related to wellness, recovery, empowerment, and client involvement are emphasized.

Hamlin FCCS

Adult Field Capable Clinical Services (FCCS) is a new Mental Health Services Act (MHSA) program that provides specialized mental health services to consumers in the field where the client is. The program is for clients who are at risk of institutional placement, have difficulty stabilizing or engaging in traditional mental health services, are repeatedly in and out of hospitals and emergency rooms, and have difficulty meeting initial goals of mental health recovery.

Target Population: 

Adults between the ages of 26-59 who have been diagnosed with an Axis 1 mental disorder who are isolated, unwilling or unable to access traditional mental health outpatient services due to various barriers toward service engagement including treatment resistance, physical disabilities, or because of the stigma associated with receiving clinic based services.

Ages of Population: 

26-59 year old adults who have been diagnosed with an Axis 1 mental disorder.

Community Assessment Services Center (CASC)

The CASC mans the Info-Line (help line) for Substance Abuse Prevention and Control as well as providing substance abuse and mental health assessment and referral services to the General Relief, CalWORKs, Department and Children and Family Services, the Probation Department through AB109, Medi-Cal and Community Clients. We also man the SAPC Info-Line and help individuals in the San Fernando and Santa Clarita Valley connect with the service providers in their neighborhood.

DVPTP provides abuse survivors risk assessment and safety planning; clinical evaluation and therapeutic counseling; case management and domestic violence psycho-education services; crisis intervention; legal advocacy and employment preparedness in individual and/or group setting.

CalWORKS Mental Health Services

CalWORKs Mental Health Supportive Services Program is an outpatient program that provides both a comprehensive and integrated array of Mental Health Services, Targeted Case Management Services, Medication Support Services, and Crisis Intervention Services to adults who experience mental health barriers to employment. The program maintains a family focus, where its mission is based upon the psychosocial rehabilitation model, in which principles related to wellness, recovery, empowerment, client/family involvement, and choice are emphasized.

 

Target Population: 

The target population for the CalWORKs Mental Health Supportive Services Program is defined by the following criteria:

  • self identification of mental health barriers during the screening process conducted by DPSS personnel
  • the client has children under the age 18
  • the client and his/her family are in need of financial assistance.

Clients present with symptoms that are characteristic of affective, anxiety, or adjustment disorders. Additionally, issues related to co-occurring alcohol/drug use/abuse and domestic violence are also assessed/discussed during the initial evaluation and explored throughout the treatment process.
