This program provides outpatient counseling services to participants with primary substance use disorders. The program will thoroughly evaluate all clients upon intake, utilizing a screening packet comprised of both clinical and case management assessment tools.

Staff will continually re-assess a client’s level of functioning and sobriety in order to gain the most benefit from the treatment program. Staff will address client’s levels of functioning, such as change in mental, emotional, physical health and wellbeing. Significant changes in a client’s condition will be immediately addressed by the program’s treatment team and referrals for necessary medical or residential detoxification services will be made with the client.

Program staff will further assist clients with improving their life functioning (home, vocational/educational, community) by assessing, referring, and linking clients to the appropriate services offered by the San Fernando Valley Community Mental Health Center’s program(s) or external resources in the community. Staff will provide clients with assistance toward accessing these services and referrals until clients successfully access and/or utilize these resources independently. All services and referrals offered to the client will be documented in a Progress Note and Treatment Plan in the client’s medical record.

If documentation, transportation, or accompaniment is required, staff will accommodate to ensure a seamless linkage.

Criteria for Admission (or readmission)

The screening, assessment and treatment planning utilizes an integrated approach which begins at the first point of contact and addresses substance use as well as a person’s level of need for an in-depth assessment. This integrated approach continues throughout treatment, measuring the participants’ readiness for change, identifying client strengths or problem areas that may affect the process of treatment and recovery, and engaging the client in the development of an appropriate treatment relationship.

What Happens at Intake?

The following information will be gathered through a comprehensive clinical interview:

  1. Social, economic and family history;
  2. Educational background;
  3. Employment History;
  4. Criminal History, legal history;
  5. Medical History;
  6. Alcohol and/or other drug history;
  7. Previous treatment

Within 72 hours of admission, each participant will attend an orientation which will describe the functions and requirements of the program and will meet individually with their case manager to establish a treatment plan.

What Makes Someone Ineligible for Services?

There is a zero tolerance for the possession or use of drugs and alcohol on program premises. Clients who are found to test positive during random monitored drug testing and/or whose behavior is determined inappropriate are made aware of our policies and procedures regarding being under the influence.

These clients are given three referrals and every effort is made to link them to other providers.

Clients who are asked to leave or are abruptly terminated due to dangerous behavior, or are in need of immediate medical evaluation, will be referred to appropriate outside agencies, and a critical de-briefing will be conducted.
