The PATH MIT program provides outreach and engagement to chronically homeless individuals that are not being served by the mental health system. The goals of the program are to assist individuals in accessing mental health, physical health and housing through intensive, field-based case management services.
The program’s philosophy adheres to the Housing First and Harm reduction approaches of providing a high tolerance, low demand environment to consumers and has no time limited engagement. Once clients are engaged and consent to services clients are immediately placed into housing and stabilization services are rendered using the evidenced based practice of Critical Time Intervention.
Services include:
- Intensive outreach/engagement and field-based services based on client’s self-determination.
- Other services provided include: mental health services, primary health services, medication support, housing location assistance, benefit assistance, money management, crisis intervention, dual diagnosis services, transportation and referral/linkage.
- Once a consumer obtains permanent housing, supportive services will be delivered in the home and in the field using the evidenced based practice of Critical Time Intervention in order to ensure retention in their new homes.
- PATH MIT will collaborate with various community partners such as health centers, law enforcement, homeless shelters, the Housing Authority, and hospitals in order to best outreach and engage consumers.
